Alcaraz suffered a slight hamstring injury in his defeat to Jannik Sinner in Sunday's final at the ATP Finals, and although he had traveled to Bologna with hopes of competing, tests have ruled him out of the tournament.
"I have an edema in my right hamstring and the medical recommendation is not to compete," he posted on social media.
"I have always said that playing for Spain is the best thing, and I would really like to be able to battle for the title. I am going home feeling sad," added Alcaraz.
